California has the strictest phlebotomy licensing requirements in the nation. You must obtain CPT I licensure through the California Department of Public Health before legally working as a phlebotomist in the state.

Issued by California Department of Public Health (CDPH). CDPH also issues Limited Phlebotomy Technician (LPT, skin puncture only) and CPT II (adds arterial draws plus experience).

Enroll in Approved Training Program
- Find a CDPH-approved phlebotomy training program
- Confirm they cover at least 40 hours didactic + 40 hours clinical

Complete Didactic Training (40 hours)
- Classroom instruction covering phlebotomy theory, safety, procedures

Complete Clinical Training (40 hours)
- Hands-on practice in approved healthcare facility lab
- Direct supervision by licensed phlebotomist or medical technologist
- Must perform real patient draws (under supervision)
- Log all procedures completed and get supervisor sign-off
Register with CDPH
- Submit training completion documentation to CDPH
- Provide proof of didactic + clinical hours
- Pay registration fee (approximately $200-300)
- CDPH will provide CPT registration number
Schedule & Pass Approved Exam
- Register for ASCP PBT or NHA CPT exam
