FTC Disclosure (16 CFR Part 255): PhlebotomySkills affiliates receive a commission for referrals that result in a purchase. Affiliates must clearly disclose their material connection to PhlebotomySkills whenever promoting our products. Example: "I may earn a commission if you purchase through my link." Failure to disclose may result in removal from the program.
US tax reporting: US-based affiliates who earn $600 or more in a calendar year must submit a completed IRS Form W-9 before payout. We issue a 1099-NEC by January 31 of the following year. International affiliates must submit IRS Form W-8BEN (individuals) or W-8BEN-E (entities). Forms are requested automatically once you cross $500 in earnings; contact affiliates@phlebotomyskills.com to submit early.
Program terms: Self-referrals, coupon-stacking, and paid search bidding on "phlebotomyskills" or variants are prohibited and void commissions. PhlebotomySkills reserves the right to reject any affiliate application and to terminate accounts that violate FTC disclosure or our program rules.
